Create an accident record
Last Updated: 09/24/2026

Record an accident as soon as you know about it. If a DOT post-accident test may be required, decide and order it first; the time limits are short.

Steps

  1. Choose Employees > New Accident Record, the New Accident quick task, New Accident Record on the Accidents list, or the employee's History menu.
  2. Employee. Start typing the last name and select the employee.
  3. Accident details. Enter the date (required), time, number of injuries, fatalities, and vehicles, and whether there was a haz-mat or gasoline spill and disabling damage.
  4. Location. Enter the address, roadway number, and a location description.
  5. Accident report. Enter the report number, officer badge, reporting agency, reporting state, and whether it is federally recordable.
  6. Conditions. Describe road access control, surface, weather, and light conditions.
  7. Descriptions. Describe injuries and fatalities, any spill, damage, witnesses, police officer information, and vehicles involved.
  8. Review. Check the summary and select Finish.

What happens next

With Show additional Tasks on Finish set to Yes, Med-Stop offers follow-up tasks such as a new test request linked to the accident or adding a document. Creating the accident record does not by itself order a test.

Find the record later in Employees > Accidents or the employee's History.
