It is a request to an applicant's previous DOT-regulated employers for their drug and alcohol testing history, made with the applicant's written consent, before the person performs safety-sensitive functions for you.

• All DOT agencies: 49 CFR 40.25 sets out what to request and from whom.
• FMCSA drivers: 49 CFR 391.23 also requires a safety performance history investigation, and 49 CFR 382.701 requires a pre-employment Clearinghouse query.
