Below are the forms you can use during the employment process:

Application for Employment

A standard application form for employment. You may select an existing applicant or print blank forms.

Employment History Verification

Used to verify the safety history of an employee with their previous employers.

Request for Information from Previous Employer

Used to document contact with previous employers.

Request for Check of Driving Record

This form helps meet driver qualification requirements by checking an applicant’s driving record.

Request for Drug and Alcohol Records

Used to request drug and alcohol testing records from an applicant’s previous employer. You may select an existing applicant or print a blank form.

Employee Eligibility Verification

The Employee Eligibility Verification (Form I-9) is required by the Department of Homeland Security – U.S. Citizenship and Immigration Services to document eligibility for employment in the United States.

Fair Credit Reporting Act Disclosure

This form helps meet the requirements of Section 604(b)(2) of the Fair Credit Reporting Act. It states that a consumer reporting agency cannot provide your information to an employer or prospective employer without your written consent. Written consent is generally not required in the trucking industry.

Driver Statement of On-Duty Hours

Commercial vehicle drivers are limited in the number of hours they may drive. This form is used to record their on-duty hours.

This checklist helps satisfy the driver qualification requirements under 49 CFR Section 391.51.

Previous Employee Safety Performance History

Prospective employers must obtain accident and drug/alcohol testing histories from all DOT-regulated employers for whom the applicant worked in the past three years. A signed release form from the applicant must be included. Employers are required to provide this information within 30 days of the request.

Certificate of Compliance

An employee certificate verifying compliance with relevant requirements.

Certificate of Driver Training

A certificate confirming completion of driver training.

Certificate of Road Test

Upon successful completion of a road test, the person administering the test must complete a certificate of the driver’s road test. The original or a copy must be kept in the driver’s qualification file, and a copy should be provided to the driver.

Certificate of Violations / Annual Review

At least once every 12 months, a motor carrier must review each driver's driving record. A note documenting the results of this review must be included in the driver's qualification file.
