Med-Stop provides forms you can print during hiring and for driver qualification files. Many can be filled with an existing applicant's information or printed blank. Open them from Reports.
Hiring

| Employment History Verification | Verify an employee's safety history with previous employers. |
| Request for Information from Previous Employer | Document contact with previous employers. |
| Request for Drug and Alcohol Records | Request testing records from previous DOT employers (49 CFR 40.25). |
| Previous Employee Safety Performance History | FMCSA safety performance history from previous DOT employers, with the applicant's signed release (49 CFR 391.23). |
| Employee Eligibility Verification (Form I-9) | Required by U.S. Citizenship and Immigration Services to document eligibility to work in the United States. |
| Fair Credit Reporting Act Disclosure | Disclosure and authorization for consumer reports. |
Driver qualification
| Form | Use |

| Request for Check of Driving Record | Request the applicant's driving record from state agencies. |
| Checklist for Qualification of New Drivers | Track driver qualification file items (49 CFR 391.51). |
| Driver Statement of On-Duty Hours | Record a new driver's recent on-duty hours. |
| Certificate of Road Test | Completed by the person who gave the road test; keep it in the driver qualification file and give the driver a copy. |
| Certificate of Violations / Annual Review | Document the annual review of the driver's driving record. |
| Certificate of Compliance | Employee certificate of compliance with relevant requirements. |
| Certificate of Driver Training | Confirm completion of driver training. |
