The Missing Documents area lists tests that have been performed but are missing documents that must be scanned into Med-Stop. Tests that were completed correctly, with all required documents scanned, do not appear here.

Documents that are commonly missing

• The medical history form, completed by hand and signed by the customer, for a medical examination.
• The MRO copy of the drug testing custody and control form (CCF).
• The breath alcohol testing device printout for an alcohol test.

Why it matters

A test record is not complete until its required documents are in Med-Stop. Missing paperwork can delay review of results and leave gaps in the records employers rely on.

What to do

• Scan the missing document and upload it to the test.
• If you cannot scan it for any reason, contact Med-Stop support to fax a copy so it can be added to the record.
