If your company uses pre-printed Federal or non-DOT custody and control forms (CCFs) for drug testing, you can order them and track the orders in Med-Stop. A form order is separate from a test request: it supplies paperwork, it does not schedule a test.

Steps

1. Open Company > CCF Drug Form Orders and review existing orders, so you do not order twice.
2. To request forms, select Company > Order CCF Drug Forms and complete the Pre-Printed Drug Test Forms Request.
3. Review the order details before you finish.
4. When the forms arrive, open the order and use Form Received. Use Cancel Order only when you no longer need the forms and the action is available.

Tips

• Store forms securely; each CCF has a unique specimen ID.
• If your collections use an electronic CCF, you may not need pre-printed forms. Check with Med-Stop Support.
