With FormFox, the federal custody and control form (CCF) is completed electronically (eCCF). Printing Copy 1 - Lab Copy is part of the specimen collection process: this copy travels with the specimen to the laboratory. The CCF, in paper or electronic form, is covered by 49 CFR Part 40, Subpart D (49 CFR 40.45).

Steps

1. In Collection Step 4, choose which CCF copies to print. Copy 1 - Lab Copy is selected automatically and cannot be cleared.
2. Click PRINT. Copy 1 prints to your default printer.
3. If an Adobe Reader warning appears, select Yes so the page prints. Do not select Do not show this message again.
4. After the copy prints, click Close. Do not click the X in the top-right corner to close the page.
5. Place Copy 1 in the bag with the specimen for transport to the laboratory.

Important: Do not omit Copy 1 - Lab Copy. Doing so may result in a fatal flaw and the test being canceled (see Subpart I).

What happens to the other copies

• FormFox automatically faxes the MRO copy.
• FormFox does not automatically distribute any other copies of the CCF.
• The donor may receive a printed copy, or provide an email address or cell phone number to receive an electronic copy.
