If you no longer wish to receive messages from Med-Stop, you may request that we do not send faxes, e-mail messages in the future. The request must be communicated to us by calling our phone number (877) 633-3633, by sending back the fax cover page you received from Med-Stop to our opt-out fax number (847) 647-6608 or by filling and submitting this form.
