Report a DOT violation to North Carolina

North Carolina requires employers to report certain DOT drug and alcohol violations by commercial drivers to the state. Med-Stop shows a state reporting alert for the affected test until you record that the report was sent. Check the agency's current instructions before sending; state addresses and numbers can change.
Print the state report
- On the Welcome dashboard, open Abnormal Urine Drug and Alcohol Tests and select the test record for the driver.
- Open the Documents tab.
- Open and print the Report of Positive Drug Test Result - North Carolina form and complete it.
Send the report
Send the completed form to:

Record that the report was sent
- On the Welcome dashboard, open Abnormal Urine Drug and Alcohol Tests and select the test record for the driver.
- On the Details tab, select Record State Report.
- In Reported to State Agency, select Yes.
- Enter the date and time the report was sent.
- Select Finish. The state reporting alert closes.
