Yes. The driver completes the health history on the Medical Examination Report and must answer completely and truthfully; the driver certifies the information is accurate, and false statements can invalidate the certificate.

The medical examiner records medical conditions even when they are not a reason to disqualify the driver, and discusses them with the driver to encourage appropriate care. Complete information lets the examiner make an accurate decision and, if needed, set a shorter certification period instead of disqualifying the driver.
