Employee Documents

Med-Stop allows you to attach and retrieve the documents in the electronic format to/from the employee record.

To create a new employee document press the New button.

You can attach any file to the employee document list, some predefined categories are as follows:

  • APPLICATION FOR EMPLOYMENT
  • EMPLOYMENT HISTORY VERIFICATION
  • REQUEST FOR INFORMATION FROM REVIOUS EMPLOYER
  • REQUEST FOR CHECK OF DRIVING RECORD
  • REQUEST FOR DRUG AND ALCOHOL RECORDS
  • EMPLOYEE ELIGIBILITY VERIFICATION
  • FAIR CREDIT REPORTING ACT DISCLOSURE
  • DRIVER STATEMENT OF ON-DUTY HOURS
  • PREVIOUS EMPLOYEE SAFETY PERFORMANCE HISTORY
  • CERTIFICATE OF COMPLIANCE
  • CERTIFICATE OF DRIVER TRAINING
  • CERTIFICATE OF ROAD TEST
  • CERTIFICATE OF VIOLATIONS/ANNUAL REVIEW
  • EMPLOYEE INFORMATION FORM
  • EMPLOYEE TRAINING INFORMATION FORM
  • DRIVERS LICENSE
