Required Items on Each Report

Each report of a split specimen failure to reconfirm must include the following information:

• MRO’s name, address, phone number, and fax number
• Collection site name, address, and phone number
• Date of collection
• Specimen ID number
• Laboratory accession number
• Primary specimen laboratory name, address, and phone number
• Date the primary specimen result was reported or certified
• Split specimen laboratory name, address, and phone number
• Date the split specimen result was reported or certified
• Primary specimen result (e.g., name of drug or adulterant identified)
• Reason for the split specimen failure to reconfirm (e.g., drug or adulterant not present, specimen invalid, split not collected, insufficient volume)
• Actions taken by the MRO (e.g., employer notified of failure to reconfirm and requirement for recollection)
• Additional information explaining the reason for cancellation
• Name of the individual submitting the report, if not the MRO
